摘要:随着我国人口老龄化加剧及饮食结构变迁,糖尿病患病率呈逐年上升趋势,血糖异常及其并发症已成为严重公共卫生问题。研究表明,膳食纤维具有显著的降血糖生物活性,不仅对2型糖尿病患者的血糖管理具有积极作用,还可有效改善妊娠期糖尿病患者的代谢状况。本文系统综述了膳食纤维的降血糖作用机制,包括延缓碳水化合物吸收、调节肠道菌群-短链脂肪酸-胰高血糖素样肽-1(glucagon-like peptide-1,GLP-1)轴、促进短链脂肪酸生成以及改善胰岛素敏感性、调节磷脂酰肌醇3-激酶(phosphoinositide 3-kinase,PI3K)/蛋白激酶B(protein kinase B,AKT)通路等方面。同时,本文还探讨了不同类型膳食纤维的有效剂量及其影响因素,为基于膳食纤维的功能性食品开发提供了科学依据。
With the intensification of population aging and changes in dietary structure in China,the prevalence of diabetes has been increasing year by year.Abnormal blood glucose levels and their complications have become serious public health issues.Research has shown that dietary fiber possesses significant blood glucose-lowering bioactivity,which not only plays a positive role in blood glucose management for patients with type 2 diabetes but also effectively improves the metabolic status of patients with gestational diabetes.This article systematically reviewed the mechanisms by which dietary fiber lowers blood glucose,including delaying carbohydrate absorption,regulating the gut microbiota–short-chain fatty acids–glucagon-like peptide-1(GLP-1)axis,promoting the production of short-chain fatty acids,improving insulin sensitivity,and modulating the phosphoinositide 3-kinase(PI3K)/protein kinase B(AKT)pathway.Additionally,this article discussed the effective doses of different types of dietary fiber and their influencing factors,providing a scientific basis for the development of functional foods based on dietary fiber.
